You may discontinue your monthly/membership EFT payment as follows:
1) Send an e-mail notice to email@example.com prior to your next monthly membership renewal date. Your automatic renewal will be prorated based on 30 day cancellation policy and then cancelled and we will process a prorated payment fee. Your access will then be deactivated to the wellness center.
Please note the following policy for corporate Boot Camp and Way To Live corporate/community Nutrition programs
A refund will be granted for cancellation prior to a program start date or event less a $30 processing fee. Should a medical emergency or death in the family arise during a session or event, you may transfer your registration to another person or you may apply registration towards a future event, less a $30.00 processing fee for a period of 1 year from date of original scheduled session date.
We do not refund payment for programs that are currently in session. However, we will defer any unused portion of your program to the next available session on a prorated basis for a period of 1 year from date of original scheduled session date. If you are absent from your program during a session you will not be credited for your missed days.
The following policies are specific to Personal Training:
Date of Completion
All sessions must be completed within One year from date of purchase. Any sessions remaining will become void. Sessions may be postponed however, due to medical reasons, or military deployment. Medical documentation and military papers must be submitted to FITC in order to have session package postponed.
Cancellation of Session (Client/Trainer)
24 hour notice is to be given in order to cancel or reschedule a session. If less than a 24 hour notice, session will be charged. If trainer fails to provide a 24 hour notice to the client, then an additional session will be added to client’s package at no charge. Exceptions may be made, but it will be at the discretion of FITC.
All sessions are to start on time. In the event client is late, trainer will wait 15 minutes. If client does not arrive within that time, session will be cancelled and charged to client. If trainer arrives late, the amount of time will be added to the end of the session for an extended session.
If for any reason client is unsatisfied with the service of their training sessions, an alternate trainer may be assigned upon request. If an alternate trainer is not wanted, FITC will refund the remaining balance of training package. Please note that FITC does not offer refunds or credits after client has participated in the first three sessions of the training package.
